There are no templates or separate Microsoft Word forms for Mentor Agreement or Project Proposal. The Formstack form contains all components you need to submit.

Signature Work Proposals submitted after the official deadline will incur an automatic administrative grade reduction to the final Capstone 495 grade. Proposals submitted seven days late will have the grade reduced by 10 points; fourteen days late will have the grade reduced by 20 points; twenty-one days late will have the grade reduced by 30 points. No grade will be reduced in this way below a 60 (D-).
